Because of some manufacturer's changes in their return policies for on-line retailers, before a defective product can be returned or replaced under warranty, you must first fill out and submit this Defective Product Report. Once we receive the completed report, if applicable, we will ship the replacement product right away.
This form is to be filled out by anyone returning a defective product for any reason. You may also need to fill out an RMA Request Form to get authorization to return the product if the product is defective after 30 days from purchase date.
